Older adults often experience diverse health issues which require complex, on-going care. In addition to home care services, Agapé Home Care provides Geriatric Care Management to support families as they and their loved ones face the challenges of aging.

Agape Home Care GCM’s assess, advise, and advocate for the benefit of older adults. GCM’s are sometimes referred to as “surrogate daughters” because they offer the same level of caring and support that a family member would. However, family members often lack expertise in older adult care and are not always available.

Agapé Home Care’s GCM’s are proficient in organizing, arranging, and supervising the care of older adults who are diagnosed with Alzheimer’s, dementia, Parkinson’s, COPD, diabetes, heart disease, cancer, or chronic illnesses that affect aging adults.
